Consent Forms
Every athlete is required to have the appropriate consent forms completed by his/her parent & turned in to their Coach or the Team Secretary. If the appropriate consent forms are not completed and ON FILE, the athlete in question will NOT be allowed to participate in said activity.
Consent forms are required for the following:
*Unrelated Adult Athlete to share a hotel room or sleeping arrangement with a minor athlete.
*Unrelated applicable adult providing local transportation to minor athlete.
*Licensed massage therapist or other certified professional/health care provider to treat minor athlete.
*Unrelated applicable adult to travel to competition with minor athlete.
PLEASE PRINT AND COMPLETE APPROPRIATE CONSENT FORM AND TURN IN TO COACH OR TEAM SECRETARY.
